Participant Applications for Summer Trips 2009
1. Step One: Chose the trip you want to apply to.
2. Download Summer Application
2. Fill Out Completely (Word Document)
3. Print Hard Copy. Sign on page two.
4. Bring to Community Service Center in Mary Graydon 273: Place in Alt Break Box by February 13, 2009.
5. You may be contacted via email by Student Trip Leaders to sign up for an interview.
6. You will be notified via email by your Student Trip Leaders by March 1st, 2009.
Application Deadline February 13, 2009. By 12:00 PM in the Community Service Center (MGC 273)
Required Forms For Participants:
These need to be filled out by all trip participants and leaders, including staff/faculty.
Please download Consent & Release and Health Insurance form for everyone on your trip. Return completed forms with signature to Shoshanna in the Community Service Center. Mary Graydon 273.
